Here are my notes about this document:
During World War I, all men in the U.S. between the ages of 18 and 45 were required to register for the draft. [Here are more details for those who are interested: On June 15, 1917, those aged 21 to 31 had to register; then on June 5, 1918, those who had subsequently turned 21; then on Sept. 12, 1918, men aged 18 to 21 and those aged 31 to 45 had to register. The war ended two months later (Nov. 11, 1918)].
Jerome's date of birth is listed here as March 26, 1886, and his age as 32. The date of birth in Jerome's declaration of intention is March 16, 1886. In later documents related to his naturalization or his death, his date of birth is March 14, 1886. The Julian calendar was still in use in the Russian Empire in when he was born; to convert a 19th century Russian date to the Gregorian calendar one needs to add 12 days (see the useful chart in Wikipedia).
So perhaps March 26 was Jerome's Gregorian birth date and March 14 was his original Julian birth date?

I believe draft cards were filled in by officials, so this probably isn't Jerome's handwriting -- but this does include Jerome's signature.
On the reverse of the form, he is described as medium height, medium build, blue eyes, brown hair, with no obvious disqualification.
On the reverse of the form, the date of registration is stamped as Sept. 12, 1918, and the registrar's signature appears to read Frederik S. May.
