Here are my notes about this document:
- Albert's occupation is "Day laborer for H. Rosenthal." Presumably this means he was working for his nephew Harry.
- Albert and Naomi's address is 99 North Milton. See this address today in Google Maps.
- The birth took place at St. Joseph's Hospital.
- The maiden name of the baby's mother is given as "Noemae Brock" (it's hard to be sure of how "Naomi" is spelled on this form, as it's handwritten, but it looks like "Noemae").
