Here are my notes about this document:
- Benjamin's birth date is the same one he gave in his petition for naturalization : Oct. 3, 1872. But his gravestone gives his year of birth as 1873, and the date of birth given in his daughter Gladys's "Schwartz-Weiss Family Tree" is Nov. 19, 1873.
- Address is 1472 Brook Ave. (the same as in the 1910 and 1915 censuses). See this address today in Google Maps.
- This confirms Gladys (Schwartz) Kaminsky's statement in her text "The Beginning of Me" that when she was born in November 1918 the family lived at 1472 Brook.
- Occupation = cloth cutter
- Employer's name = J.C.L. Murphy
- Place of employment: 7 W. 30th St., New York. See this address today in Google Maps.
- His name is given as Benjamin Schwartz (not "Beny" or "Beni" as we saw in documents of the 1890s), and this is also how he signs.
- Nearest relative = Julia Schwartz (his wife).
- He is described as having grey eyes and grey hair, and being partly bald. (Strangely, this last fact is recorded in the section for "Has person lost arm, leg, hand, eye, or is he obviously physically disqualified.")
