JACOB WEISS - BIRTH - JANUARY 13, 1883 - MANHATTAN

This is the birth certificate of Jacob Weiss, the younger brother of Julia Weiss and includes information about his (their) parents.
Document received by mail from New York City Municipal Archives, March 2016.
Jacob was the fifth surviving child of David Weiss and Helen (Greenbaum) Weiss. He was the first in the family to be born in the U.S. (his older siblings were born in Hungary.)
The date of birth in this certificate, January 13, 1883, is the same as in Jacob Weiss's 1918 draft registration.
Place of Birth: 66 Stanton St. Probably this address (on the Lower East Side of Manhattan) is where the family lived. See 66 Stanton St. today in Google Maps.
It gives the following information for the mother: Name of Father: Elonora Weiss; Maiden Name of Mother: Elonora Grünbaum; Birthplace of Mother: Hungaria; Age: 35. [We know her as Helen Greenbaum (from Gladys (Schwartz) Kaminksky's 1980 family tree.)]
It gives the following information for the father: Name of Father: David Weiss, Birthplace of Father: Hungaria; Age: 38; Occupation: Tailor.
It says: Number of Child of Mother: 10th; How many of them now living: 5th. This supports information in Gladys Kaminsky's 1980 family tree, where she wrote:

There had been other babies before [Julia], but they got sick in their second summer. Her mother recognized the deadly symptoms and ran with each to a photographer to get a picture before they died. Helen Heyman remembered seeing a mantle with six or seven of those tragic pictures. Only her [Helen Heyman's] mother, Rose, survived, until Julia was born.
For "Name and address of Medical Attendant or other Authorized person, in own handwriting" it says: Rosa E. Hacker, No. 2 Ave B cor Houston. See 2 Avenue B (which is indeed at the corner of Avenue B and East Houston) today in Google Maps. (According to Google Maps it's an eight-minute walk from 66 Stanton St.

Presumably Rosa E. Hacker was a midwife? Actually there is a listing for her in the 1884 New York City directory: see weiss-jacob-birth-nurse-in-1884-NYC-directory.jpg -- it says "Hacker Rosa E. nurse, h 2 Av. B". The "h" stands for "home".

The handwriting on the rest of the form looks similar to the handwriting in the part about the "Medical Attendant", so probably the nurse filled out the whole form.
