- Employer's name and address: Cooperative Press, 15 Spruce, New York. See this address today in Google Maps.
- Nearest relative: Mrs. Hannah Weiss, same address as Jacob = his wife.
- Under "Has person lost arm, leg, hand, eye, or is he obviously physically disqualified." it says "Three fingers on right hand missing." This injury was also mentioned in the 1906 orphanage application of his younger half-brother Edward, which said "Jacob lost 3 fingers in accident."
